About the Role
As a Senior Occupational Therapist within the Integrated Care team at CALHN, you will play a key role in delivering high-quality, person-centred therapy services to patients across both clinic and community settings. You’ll be instrumental in providing comprehensive assessments, targeted interventions and effective care coordination to support hospital avoidance, early discharge and hospital substitution programs. With the flexibility to work across multiple sites, including BRIGHT clinics, Hospital in the Home, and other CALHN services, this role offers variety and purpose in supporting diverse patient needs.
In this dynamic position, you’ll use your well-developed clinical skills to manage complex cases involving physical, mental and psychosocial challenges—always with a culturally responsive and collaborative approach. As a clinical leader, you’ll mentor junior staff, supervise students, and contribute to the development of service delivery through your advanced practice and problem-solving abilities. Your knowledge of disability, community resources and referral pathways will be vital in helping patients achieve safe and sustainable outcomes in their recovery journey.
