Qualifications:

• Valid Physical Therapist license in the state of Colorado.
• Compassionate and patient-centered approach to care.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.

Responsibilities:

• Provide exceptional home health physical therapy services to patients in the Commerce City and Brighton areas.
• Develop and implement personalized treatment plans based on patient needs and goals.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
• Maintain accurate and up-to-date patient records.

Job Types: Full-time, Part-time, PRN

Pay: $72.00 - 76.00 per visit
